Pick a heel and a top design, then add accessories and you'll be done! The other mode is called "Dream Shoes". Your objective in this mode is to recreate the example shoe correctly. When you start, you'll see a preview of the shoe you're trying to recreate. Don't worry if you forget the details! You can check the shoe anytime you want.
